1. Applicant must be an employee of Guardian Life of the Caribbean (GLOC) for the duration of the membership and the Applicant and dependents must be insured under the group health plan for GLOC employees.

2. The health insurance policy for the Applicant and dependents must not contain any exclusions which pertain to the services and benefits covered by the ACI Heart Net Plus Membership namely exclusions for diseases of the cardiovascular system and heart or similar.

3. The Applicant agrees that membership to the ACI Heart Net Plus program is an optional additional benefit to their GLOC group health insurance plan. This program is offered by an independent company and the applicant understands membership is voluntarily and that they are under no obligation to enroll.

4. The Applicant agrees to notify the Company in writing as soon as possible if any conditions change related to these Terms and Conditions which would materially affect the Applicant or his dependents ability to meet the requirements for membership.

5. The Applicant agrees that once enrolled in the program that the first membership card will be provided at no cost but should a new card be required for the Applicant or dependents thereafter it would be at a cost to the Applicant.  The current replacement card cost is $25 per card but is subject to change without notice.

6. The Applicant agrees that the list of benefits outlined in clause 15 of this agreement is subject to change at any time during this agreement with one month’s notice being sent to GLOC.

7. The Applicant agrees that membership card(s) must always be presented whenever Applicant or Dependents are accessing benefits under this program and that if requested the Applicant or Dependents will provide ID for confirmation of identity.

8. The Applicant agrees that in order to access the benefits provided by the ACI Heart Net Plus program all appointments must be scheduled through the customer service representative for the ACI Heart Net Plus program.

9. The Applicant may choose to apply for a deferred payment plan for any insurance co-payments as one of the benefits available under this program. If the deferred payment option is utilized, then the Applicant agrees to sign a promissory note for the full amount being deferred and to arrange for payroll deductions and direct payment to the Company from GLOC in accordance with the payment schedule approved by the Company.

10. The Applicant understand and agrees that all of the doctors in the ACI Heart Net Network are independent medical doctors who are registered with the Medical Board of Trinidad and Tobago and that they are not employees of the Company. Hence the Company in no way accepts any liability for the advice given or services performed by the doctors. The Applicant therefore agrees to hold the Company harmless from any loss, damage or personal injury resulting from the actions of the doctors who are acting as independent medical professionals.
    
    Membership Term and Termination

11. The Initial membership once approved shall be for a period of one year with the expiry date listed on the membership card. 

12. The Company may terminate membership with one month prior notice to the Applicant at which time all balances on deferred payment plan arrangements made under clause 10 by the Applicant will immediately become due and payable unless a separate agreement is entered into between the Company and the Applicant.

13. The Applicant may terminate membership with one month prior notice to the Company at which time all balances on deferred payment plan arrangements made under clause 10 by the Applicant will immediately become due and payable unless a separate agreement is entered into between the Company and the Applicant.

14. Membership may be terminated without notice if one or more of the following occur:
    a.    The Applicant fails to meet any of the requirements listed in item 1 and 2 above. 
    b.    GLOC notifies the Company in writing to discontinue the membership for whatever reason.
    c.    The Applicant fails to comply with clause 4  
    On termination of membership all balances on deferred payment plan arrangements made under clause 10 by the Applicant will immediately become due and payable by the Applicant or legal personal representative unless a separate agreement is entered into between the Company and the Applicant. 
    
    Membership Benefits

15. The current schedule of benefits for approved members of the ACI Heart Net Plus program is attached as Schedule 1 to this agreement subject to changes under clause 6 of this Agreement.
    
    Notices

16. Any and all communications in regard to this agreement shall be delivered either personally to the address/email provided or by registered mail to Guardian Life Of the Caribbean. All member notices will be delivered by forms deemed necessary by Advanced Cardiovascular Institute Ltd. 
    
    Assignment

17. Without prior written approval no portion of this agreement may be transferred or sold to any other third party.   
    
    No Warranties

18. No warranties are given or implied by this membership agreement.
    
    Effectiveness and Interpretation of Agreement

19. This agreement shall be set in the jurisdiction and under the Laws of Trinidad and Tobago. This membership agreement sets forth the entire understanding of the above mentioned agreement and unless otherwise stated is not inclusive of any oral or prior written agreements. Any modifications or changes to this agreement must be in written form and signed by both parties.

Schedule 1 - Benefits & Discounts

1. Concierge scheduling of appointments with ACI Heart Network of primary care doctors and Cardiologists which is accessed by calling the program Customer Service Representative.

2. Annual Heart Disease Risk Assessment overseen by ACI Nurses.  As part of the assessment the member will get a discount voucher to do a cholesterol test by a third-party service provider and will have to pay for the discounted test in order to complete the assessment.

3. Voucher for copayment for first Cardiologist consultant if referred from primary care doctor after Annual Heart Disease Assessment.  Only valid for first visit to Cardiologist and does not include any additional medical tests done by the doctor.

4. 20% Discount on ACI Technical Fees for angioplasty or PCI if medically required by Member

5. 30% Discount on Boston Scientific Promus Elite Stents if medically required by Member.

6. 20% Discount on Boston Scientific Synergy Stents if medically required by Member.

7. 20% Discount on ACI Technical Procedural Fees for ICD or CRT-D devices.

8. 20% Discount on Boston Scientific CRT-D Devices if medically required by Member.

9. 20% Discount on ACI Technical Fees for Electrophysiology Studies w/ Ablation if medically required by Member.
